Stockholders and Spouses

1. Free board and lodging up to suite room accommodation

2. Fifty percent (50%) discount of the following :

a. Professional fees on house case rates and OPD Consultation

b. X-ray, Laboratory and Rehabilitation Procedure

c. Cardio-pulmonary services and ECG

3. No operating room, delivery room and emergency room fees

4. Discount for pharmacy, CSR, OR materials computed at Cost of Acquisition plus 12% (EVAT)

5. For dental services:

a. Free consultation

b. Thirty percent (30%) discount on Restorative, Preventive, Periondontics, Endodontics,

Surgery and Bleaching

c. Ten percent (10%) discount on Prosthodontics procedures

6. For Medical Stockholders (after payment of RIGHT TO PRACTICE)

a. May hold clinic within the hospital for a maximum of 8 hours a week paying only the

maintenance, water & electric bills; and

b. Will be included in the decking / rotation of House Cases
